You should ask for a review if there has been a significant change or there has been further deterioration in any of the following areas:-
- Behaviour
- Cognition (understanding)
- Emotional needs
- Communication
- Mobility
- Nutrition (food and drink)
- Continence (toileting)
- Skin (disorders/breakdown)
- Breathing
- Drug therapies and medicationMini strokes/funny turns
Or the individual’s needs have become more:-
- Intense
- Complex
- Unpredictable
You should ask a registered NHS practitioner involved with the individuals care to complete an NHS Continuing Health Care Checklist to see if the individual could now be eligible for NHS funding. If you don’t have an NHS professional involved, contact the local Clinical Commissioning Group to request an assessment or review.
ACT NOW if you are challenging a decision that has already been made. You only have 6 months to notify the CCG.
